Yes. I voted on a bond issue in North Dakota before graduating high school.

In September of 2002 I thus became one of the very few 18 year olds who could honestly make this claim.

Not yet. I voted in New York in 2008, but now I regret not having completed changing my voter registration to Massachusetts earlier to vote in Tuesday's senate race.

Yes. MN, MA, and IL. In two different states I was able to vote for the US Speaker of the House.

I voted in four non-contiguous states within a 10 year timespan: MN, NY, VA, and WA (in that order).  I also lived in 5 US states and 1 Australian state in a 13 year timespan.

Yes, I briefly deregistered myself in Virginia to vote in Washington state after starting my first year at the University of Washington.  A year later, I changed my mind and re-registered myself in Virginia.
