Johnson is now on every ballot except Kentucky and Rhode Island, both of which are due September  9. Stein has missed IN, NC, OK, GA, NV and SD.

The libertarians don't list themselves as on the ballot in Connecticut or New Hampshire. I think they're just waiting for official notification, but still.

This is the official libertarian ballot access map:



Connecticut is confirmed by the SoS, not sure why they don't list it. You're right for New Hampshire, it has not been confirmed yet, but they turned in over 6000 signatures and only need 3000 valid ones.
