Could prayers for the dead be included though, as bits of the Church of England (who, in theory, though perhaps not in practice fall under the Protestant umbrella) indulge in them, and, I believe, Martin Luther himself had a favourable opinion of them in the case of dead infants. Are you planning to offer more than two options on any of these?
Yes. I'd like to have a clear cut set of answers for as many questions as possible. e.g. The answers to the church governance one would go something like: Episcopal, Presbyterian, Congregational (Elder-led), Congregational (Democratic).
Obviously that won't work for some questions, so in those cases I'll turn it into a "Strongly Agree, Somewhat Agree..." in response to a proposition.
Also I think something about Sacraments would be a good addition to the high/low spectrum.
How much diversity is there within Protestantism about the number of sacraments? With the exception of a few Anglo-Catholics, the view is communion/baptism only, is it not?

Within the context of Protestantism free will is generally thought of as 'liberal'.
