Yes, it counts the parishes of Louisiana as counties.

I guess counties with no government are counted, and so are cities that are in no county. I don't think Alaska Boroughs (of which there are 16, plus the "unincorporated borough") are included. Also, there's a bit of Montana (the part of the state in Yellowstone NP) that's not really in any county.
