Regionalism is funny. Paladino leads 55-36 in "Western NY" (Buffalo area, maybe with Rochester, not sure of SUSA's definition), yet is doing worse overall than DioGuardi, who trails 34-58 in Western NY but leads narrowly in the NYC suburbs.

Also, all three Democrats are clearly understated in NYC, which seems to be a standard aspect of NY polling, not sure why.

How so?  68-20 is not a blow out enough?  New York City is more than just Manhattan.  If Staten Island and the outer parts of the outer boroughs turn out more than the rest, that's plausible - if not lowballing Republican chances.
