Are there genuine issues with water quality in the U.S, or parts of it?

In some parts, perhaps, but distrust of tap water has been a theme in the States for at least a couple of decades.  I recall in South Carolina that the real difference between tap and filter water was the temperature.

There are no reservations about bathing in it, or washing ones clothes with it.

We're a big country, so yeah, some places have relatively sh**tty water, but for the most part (and we're talking 99.7% here), there is nothing wrong with tap water in the US.

Yet distrust is absolutely widespread. Even in NYC, which has literally the highest quality tap water of all major cities in the entire world, a ton of people won't drink the tap water for fear of "getting sick" or because it "tastes different".

This is what interests me. Why such a widespread paranoia?
