I'd never want to see a candidate hurt, but, let's say Ron Paul severely injured his arm from that handshake - would it have an affect on Santorum's numbers?

Only if there was proof it was done malisciously, so no, I don't think you'd see him hurt more than maybe .5%, if even that.
