Unaffiliated Baptist.  I am not opposed to the Southern Baptist Conference, but I have attended the same church for my whole life and like it. 

With regards to my religious views, I believe that God created the world and our sin as humans has separated us from God.  Our own works cannot fill the gap we have created between man and God, so Jesus died on the cross to pay for our sins. While it may not be 100% perfect, the Bible is the best instructional book for living our lives and recounting what Jesus has done.  I know that is a tad longer than 2 sentences, but I think that covers it as far as the most basic theology is concerned.

For worship style, I prefer a combination of music and prayers aloud along with a sermon that forms the majority of the worship service.  Personally, I have always enjoyed Bible study more than actual worship services as that is when one can get into the nitty gritty of the Bible rather than just taking the word of the pastor.

Honestly, I do not know.  I understand that the scientific consensus is evolution and an old Earth, but a supernatural God can work outside natural processes.  If I were to say what my opinion is closest to, it is probably something like Old Earth Creationism.
