Best-N64
Worst-that's tougher.  My first thought was the stick and button Atari 2600 controller, but it's simple and effective (and durable).  So I'm going to go with the paddles for the Atari 2600.  One side or the other always seemed to be "shaky" and most of the games designed for them sucked.
