They factor in general living conditions with the gender specific, so I will go with Afghanistan - North Korea if you are a (straight) male. While they are obviously homophobic there wold be quite a few other countries that are more intense in their persection of gays than North Korea, so that might be worth taking into consideration for some, still hard to beat NK for general horribleness.
