2013 and beyond.

Based on mid-year 2013 estimates, Gilbert, AZ; Richmond, VA; and Boise City, ID have entered the Top 100, while Spokane, WA; Rochester, NY; and Birmingham, AL have left.

this makes Seattle, WA and Boise City, ID singular cities, and eliminates Birmingham.

Using the 4 Census Bureau divisions:

South: 39 top 100 cities
West: 37
Midwest: 17
Northeast: 7

Half of the largest 100 are in 6 states:

California (17)
Texas (13)
Arizona (7)
Florida (6)
North Carolina (5)
(tie) Nevada (4), Virginia (4), and Ohio (4).

and the trend is likely to continue.

Largest city not in Top 100 (2013):

Birmingham, AL (101)
Des Moines, IA (104)
Little Rock, AR (118)
Salt Lake City, (124)
Providence, RI (134)
Jackson, MS (138)
Sioux Falls, SD (147)
Bridgeport, CT (172)
Columbia, SC (192)
Fargo, ND (237)
Manchester, NH (248)
Billings, MT (256)
Wilmington, DE (470)
Portland, ME (519)
Cheyenne, WY (558)
Charleston, WV (725)
Burlington, VT (less than 50,000 does not qualify for Top 745).

Barring government re-organization, my guess for a largest city to enter the Top 100, would be Sioux Falls, or a city in South Carolina, Utah, or Alabama, not necessarily Columbia or Salt Lake City or Birmingham.

Plausible changes:

Nashville-Davidson (634K) swaps with Memphis (653K) as largest in Tennessee.
Mesa, AZ (458K) surpasses Tucson (526K) as 2nd largest in Arizona.
Tampa, FL (353K) surpasses Miami (418K) as 2nd largest in Florida.  Orlando (255K is longer term possibility).
Jersey City, NJ (257K) swaps with Newark (278K) as largest in New Jersey.
Chesapeake, VA (231K) surpasses Norfolk (246K) as 2nd largest in Virginia.  Richmond (218K is also a possibility.
Columbus, GA (110th) has chance to return as 2nd city,
Overland Park, KS (132nd) has a chance to become 2nd city.
Salem, OR (152nd) or Eugene (155th) have some potential to become 2nd city.
Spokane, WA (102nd), Tacoma (107th), or Vancouver (145th) could become 2nd city.
