The Papal States should be reunited, of course.

No.

I don't think the Vatican wants the responsibility of cleaning Rome's sewers and of disposing of Rome's thousands of tons of garbage every day.

Secular governance over a large area isn't and shouldn't be the Church's problem any more.

No, but the Vatican should be returned to Italy.

This, as an autonomous region.

Why would you need an autonomous region for basically just the pope alone? It should just integrate into Prati or whatever other borough/district/quartiere in Rome is next to it.

Even if Italy did reclaim the physical territory of the Vatican City State, I feel like the Holy See should remain sovereign because in its sovereign capacity it's intermittently quite useful in world diplomacy. I see no reason, however, why the Holy See and the Knights of Malta have to both be sovereign.

Agreed. If my pastor's house isn't an independent country, there's no reason for the Pope's to be either.

Does Christ not call us, as Christians, to care for the lowest of the low, even the "garbage" of humanity?

And yes, of course, as should the rest of Europe, and, by proxy, the Western hemisphere.

That sounds like a Medieval II Total War campaign gone horribly wrong.
