I can't think of any that I regularly mix up now.  At first, I got Scott and Napoleon mixed up (with their red Connecticut avatars). 

I still might occasionally get confused by avatars, at least if people change them.  I did briefly think Antonio was windjammer.  Also, a while back, TNF had a temporary Louisiana avatar, and I briefly wondered what had happened to Miles.

I've mixed up King with Vosem lately, now that both are R-IL.

I've also mixed up the names IndyTex and IndyRep just as I've scrolled around the site.
