Inspired by other geographic extreme threads, and my personal love of mountaineering:

For me it's Pikes Peak, Colorado at 14,115 ft (4,302 m), although we drove to the top. The highest I have reached by actually climbing was Mount Baker, Washington at 10,781 ft (3,286 m).

Sandia Peak, Albuquerque, New Mexico - 10,678 feet above sea level.  I have taken the tram leading up from the base of the mountain and my family who lives on the east side of ABQ have driven us up there.

Real answer: Probably somewhere on Mt. Rainier, Mt. St. Helens, or Lassen Peak.  I got closest to the summit and felt the sickest on the last one, so probably that.

I know I've been on top of Pike's Peak, not sure I've ever been higher, but did spend a couple of vacations in the mountains so may have beaten that, but I don't remember anything specific.

I guess the Rocky Mountain National Park road is the highest for me, although by foot I hiked up to Kearsarge Pass in the Sierras, which at 11,700 feet is a close second.

Went to Yosemite in 2011; absolutely gorgeous. First time I'd really ever been in the mountains. Anyways, we hiked up to Glacier Point to see Half Dome and the falls and the valley below.

Glacier Point- 7,214 ft.

Plane: ~10.000 meters or something (on my flight to the US)

By foot: Großglockner (highest Austrian mountain, which is right at my doorstep) - 3.800 meters

I've climbed that one as well! 

I reckon that the highest spot I've been to was a saddle point on the Inca trail between Sacsayhuamán and Machu Picchu.  About 4300 meters as I recall, or about 14 thousand feet

Pikes Peak in July 2007. Kind of a hazy day, but I mean you could see for miles and miles. Also got up there and felt like I had had one stiff drink on an empty stomach, so that's what's meant by "mile high." Just a light buzz. But I loved that area. I was at Bryce Canyon for some hiking as well. Utterly beautiful.

When I was attached to the 10th mountain division, I had to climb a mountain in Alaska, don't remember which one, just that it was really freaking cold.

The highest point I've reached on foot is the summit of Nevado Pisco in Peru, at 5,752 m (18,871 ft), which is above all of Europe and also above the 48 continental US states. I don't know what the highest point I've reached driving is, but it's significantly lower. I'm very interested to see if anyone on the forum can top this.

Probably Cerro Bravo in Colombia - 3985 m. It was a good hike but a really fantastic experience.

Culminates 291m but the antenna alone is one 100m, then I was like 190m, you walk on the glass above the empty, while I use to get some kinds of vertigos when a bit too high, here I felt totally secure.

But, nah, I went to see a Tour de France finish to Plateau de Beille, Pyrénées, once, between 2000m and 2200m.

And well, plane in French sky.
