Family has been traditionally either Lutheran or Episcopalian on my father's side, or Shinto-Buddhist on my mother's.  

By the time I came along, my family had stopped going to church altogether -the last time they attended as a family was when my sister was a child.  So I was never baptized.

Was baptized in a United Church of Christ that I don't have any memories of as we moved to South Carolina when I was four years old. Spent my formative years in the United Methodist Church. (No UCC church in the town we moved to.)

Mainline Protestant-ish, but I only remember going to church starting at the age of 10 after my dad's mom died.  My dad grew up nominally Methodist, while my mom was raised (Irish) Catholic, though she always loathed Catholicism.

Catholic, made my Communion and Confirmation, but other than that rarely went to Church other than Christmas and Easter up to that point.  Since my Confirmation have very rarely been to Church including Holidays, nor have my Parents.  Don't think they have been to Church since prior to them moving to the Carolinas (and that was in 09).  Last time I was at Church other than Wedding or Funeral service was probably the early 2000's

My mother's family is Southern Baptist while my father's family is Sunni Muslim. But I very much identify as a Muslim

Are we using conservative/liberal or low/high to differentiate between Evangelical and Mainline?

To answer the OP I was raised in a conservative Presbyterian congregation (PCA) and still affiliate with them.

Me: none, except my mom took me to a UU church for like a year or two sometime in the late 90s. So none.

Parents: both Catholic

Grandparents: 3 Catholic, 1 Episcopalian

Great-grandparents: 5 Catholic, 3 Episcopalian

And so on from there. Basically mirrors American religion to an extent; Mainline Protestantism started dying after the Great Wave of immigration and Catholicism started dying after Vatican II.
