It appears Carson didn't hear his name or perhaps didn't understand the procedure. Remember at one point during the debate he said "you know, when I wasn’t introduced No. 2, as was the plan, I thought maybe he thought I already had dropped out."...but he actually was introduced second, following Chris Christie.  Perhaps the cheers for Christie drowned out the sound of the moderator and he simply couldn't hear his name.   As for why Trump stood next to him, maybe he was just being nice?

I do blame ABC, because whenever the woman moderator tried to say someone's name as they were coming out it was is so muffled and quiet. But Ben is the one who made it super awkward.

Let's take a moment and acknowledge Trump's class in this situation. He knows it probably looked bad for Carson, so he stood by him to comfort him in his time of need and weather the fallout.

Trump definitely showed humanity and class in that moment, compared to all the other heartless jerks that strolled past them, probably ecstatic that they looked foolish.

Trump, and Kasich, looked just as awkward, you looser. Call a spade for a spade. Something no Trump supporter has ever been able too. They called out Trump's name several times as well.

When the moderators called a name, and the candidate did not come out, why didn't they react to that, and ask someone to escort the candidate out, and that there must be some sort of glitch? That was really poorly handled.

FWIW: Stephen Colbert did a hilarious parody of Carson's entrance.
