In 1896 William Jennings Bryan was nominated by both Democratic (with Arthur Sewall as his running mate) and Populist (with Thomas E. Watson) parties. According to the wiki, he won 149 electoral votes as Democrat and 27 as Populist, but I can't find anywhere which states he carried under which party.

According to the Atlas, there was no state that had a majority of Populist electors, but in a few it was split evenly between Democrat and Populist.

Interesting map of potential Populist support in case of a non-merger.

You can check Wikipedia's PV by state. It should have separate tickets for the two.
