Yes it is not the best thing to say right now when health talk is popular, but he's clearly referring to a situation where Clinton is already President.

In such a case, I believe Kaine would pick a VP running mate, as he would have to be comfortable working with that person.

If this is anything, maybe it's a sign that the VP nominee might be taking over campaigning for a little while. Or maybe it's nothing. I doubt they're going to replace Clinton over pneumonia. Her supporters (and that include people only grudgingly voting for her) should be supporting her now, not psyched at the prospect that she might be replaced at the last second, one last shot for Bernie. No, it could just be that Kaine is capable to the unique rigors and seriousness that is the President of the United States, one of the most important jobs in the world.
