If Hillary has to resign for health reasons during her first term and Kaine would take over, how are the prospects for 2020? Any ideas?

Well, the GOP would make the argument that Hillary, and thereby the Democrats, lied their way to the Presidency. "See! She hid her medical problems from you! Democrats cannot be trusted!" Absolutely nothing would happen in Congress since executive and legislative branches would be hating each other. The Dems have a bloodbath in the midterms if Hillary resigned by that point, but their prospects would be looking better for 2020 since Kaine isn't that bad of a candidate.
Here's what I think Kaine would perform versus a general Republican.

However, the GOP primaries could give the country anything from a boring moderate to a person who makes Trump look like a boring moderate. Here's three possibilities based on different GOP candidates.

Kaine vs Cory Gardner (or some other young conservative like Rubio, Cotton, or Ernst)

Kaine vs Kasich or some other moderate (or Jeb? This seems to be a dying breed of Republicans)

Kaine vs, OMG NO! Trump (which could totally happen)

I have no idea how this scenario would turn out due to Trump's... strange ability to make logic stop working.
