Do American elections always have these sort of cases during the actual campaign?  Like this is the second one I've watched this closely and I'm sure that in 2012 there weren't any credible cases of voting fraud before polling day.

One thing that's important to stress although clearly not about these cases: the vast majority of cases of reported voting fraud are clerical errors and no fraud was ever committed.  Also even when people vote twice its usually not some organised scheme to elect people: one thing I read from Australia was that in some of the seats that they examined for "fraud" the only cases that came out that weren't just clerical issues were just confused elderly people who accidentally voted twice: they voted early at their nursing home (compulsory voting remember; that's why they do stuff like that since you kind of have to let people who might not be able to get to the polling station physically a chance to vote) and then on polling day their family would take them out to go and vote with them, not knowing that they'd already voted early.  Other than that its people like these clowns who're voting twice either in some effort to just discredit the system for some reason, or probably more likely they've been convinced that there really is some big scary effort to steal the vote so they're going to vote multiple times to counter it; which shows how harmful that sort of reporting can be.  I mean, if you had the power and the means to organise a way of stealing an election (which would require thousands of people at least to have a decent chance of working) then why bother with the risk of organising double voting; just stuff the boxes or report false numbers instead...
