Pretty much impossible unless you have the boundaries of the US be considerably different.  The combination of the Hudson and Mohawk rivers gives NYC an advantage in connecting to the Great Lakes region over any other Northeastern port.  Unless part of that pathway remained in British control after the revolution, or contrariwise the revolution failed or was able to include Canada so that access via the St. Lawrence River valley was an easier solution, geography inevitably leads to NYC being the largest East Coast port.

I hadn't considered the Gulf Coast but perhaps you see some sort of diplomatic incident cause a war in the 1870s. The Brits gobble everything north of PA? If you wanted to go with a later POD.

Chemung Canal?

A canal that connected the Susquehanna River to the Erie Canal and Great Lakes (together with Seneca Lake and the Seneca River). It was built shortly after the Erie.

Technically doable, but why would it happen? I can't see New York State building the Western Eire Canal without building the Eastern Eire as well. My point that you'd need considerably different political boundaries still stands.

Spain joins in the French-Indian War and loses Florida. Miami becomes the South's New York City as George Washington, Baron Von Steuben, Nathaneal Greene, Henry Lee III, and Horatio Gates split. Washington, Greene, and von Steuben attack New York City, while Lee and Gates are ordered to keep the twelve thousand British troops in Miami. Miami thus develops a powerful finance and trade industry, soon being called "America's Venice." Once New York City falls, the American military finally takes Miami, forcing the British out of their last stronghold. The Treaty of Venice dictates that the British recognize American independence and pay a large amount in reparations to the Americans, the French, and the Prussians who fought with the rebels in this timeline.

From there, Miami becomes the meeting place of Napoleonic France, American culture, and colonial life. It, New York City, and Baltimore are considered the greatest ports on the East Coast, and Miami is considered the greatest of these with a population today of almost fifteen million.

The Port Authority of New York and New Jersey manages ports in both New York City and New Jersey. I'm guessing the biggest East Coast port is now in New Jersey.
