Alabama, Kentucky, Maine, New Hampshire, Utah, Vermont, Nebraska, and Wyoming are all empty, I think.   
Alabama: Soup18 or whatever that fundementalist's name is, I think.
Kentucky: Bandit
Maine: Kevinstat
New Hampshire: NH Politico, Scoonie, Badnarik 04 (or however it went)
Utah: That crazy Lephi guy who went psycho on me for some reason.
Vermont: Hmm.... I think Red NJ Avatar lived there for a few weeks.
Nebraska: Somebody, but I forget their name.
Wyoming: Cowboy.

Machiavelli is from AL.  I can't think of anyone actually from DE, HI, IN, KY, MT, NM, ND, OR (not since Bob), VT, WV or WY

Isn't Cashcow from Delaware?

No, his current name 'King of New Jersey' should be a clue to where he's really from.  He had the Delaware avatar because he felt sorry for the most neglected state in the union.

Also in protest against the North Jersey Empire.
