  It was the same John Anderson who ran in 1980.
  There were several attempts by die-hard Anderson supporters to get him to run again in 1984.  Despite those attempts he declined and subsequently endorsed Mondale.  His supporters had already gotten his name on the ballot in Kentucky, and it remained there despite his unwwillingness to run.

I'd imagine that it didn't take much to make the ballot in Kentucky in 1984, since 9 candidates were on the ballot there.

Not as much as 12 candidates on the ballot in Colorado in 2004, where you made the ballot if you paid the fee.
