...but im curious...

does anyone have a list of counties (assuming there are any)....that voted for mcgovern, carter (twice), mondale, dukakis, clinton (twice), gore  and then bush in 04.

None. The only ones that come close are Morgan, Kentucky and Cottle, Texas; both of them voted Democratic until they switched to Bush in 2000.

okay are there any counties that went for nixon, ford, reagan (twice), bush (twice), dole, bush and kerry in 04?

I forgot about that. Also note that Kerry lost Monroe, Pennsylvania by only four votes (out of 56,342 cast); that would have met your criteria.

There's a few even if you add Goldwater.
Albemarle Co VA, Danville city VA, Alpine and Mono, and IIRC one in Colorado, plus one that went for Perot in 92 but otherwise fulfills the criteria. I'd have to check to see whether my Colorado recollection is correct, though.
