None. The only ones that come close are Morgan, Kentucky and Cottle, Texas; both of them voted Democratic until they switched to Bush in 2000.

Mono and Alpine, California. Kerry was the first Democrat to carry those counties since FDR in 1936.

I forgot about that. Also note that Kerry lost Monroe, Pennsylvania by only four votes (out of 56,342 cast); that would have met your criteria.
