In 1968

Governor Nelson Rockefeller of New York defeats former Vice President Richard Nixon to win the Republican Presidential nomination.  Rockefeller selects experienced and respected corruption fighting Senator John Williams of Delaware for Vice President.

In a surprise upset, former Alabama Governor George Wallace wins the Democratic nomination over Vice President Hubert Humphrey.  Wallace selects retired Air Force General Curtis LeMay of Ohio for Vice President.

There are no independent or third party candidates.

Might I add that Wallace picks LeMay for Vice President as all prominent mainline Democrats turned down Wallace's offer to be on the ticket.
