Yes. Requiring the states to pay for some of it, and manage it, leads to hurting state budgets, inefficiency, inconsistency, confusion, etc.

Yes, Medicaid rules and regulations should be universally applicable and not subject to change with your Governor's outrage du jour.

Yes, and it should be required by law to use a reimbursement schedule that is comparable to Medicare's.

The whole reason Medicaid is such a crappy coverage system is because states refuse to put any money into it and as a result, the reimbursement rates are so low that hardly any doctors or facilities will accept it.
