well, it's not really his government that's the problem...

Summed up in a nutshell, Jean-Bertrand Aristide was elected President in 1990, forced out in a military coup in 1991 and until 1994 the country was ran by a military junta headed by Raoul Cedras. Pressure from the Clinton administration forced Cedras to abdicate and Aristide was reinstalled. Democracy was getting on track until a bunch of elements in the military got upset with Aristide and formed a bunch of militias and rebel groups that collectively became known as something like the National Front for the Liberation of Haiti. In 2005 they were able to seize control of most of the country and force Arisitide to flee. As Chief Justice of the Supreme Court Alexandre was next in line to assume the presidency, after he was sworn in the rebel groups agreed to a cease-fire and allowed UN peacekeepers into the country, but still control much of the countryside.

So while the official head of state Alexandre is mostly only a symbolic figure, the effective government is headed by militia leaders like Louis-Jodel Chamblain, which many human rights groups consider to be one of the top war criminals at large, and Guy Phillipe, a military officer who says the person he admires most is Pinochet. It's these guys that are the source of the lousy ratings mostly.
