There’s a good chance McCain himself wouldn’t have ran again in 2012. He’d be 76 by then and the 2009-13 presidential term was far from easy. My guess is Palin would be nominated as a sacrificial lamb or someone like Romney, Pawlenty, or Huntsman would be nominated.

This.  2010 and 2012 with President McCain (and consequently, little or no stimulus) would be the best Dem wave elections since the 1930's.  They could already be near 2/3rds in both houses of Congress going into 2012.

The presidency takes a toll on the incumbent, so there’s a scenario where incumbent President Palin runs in 2012 with the Republican establishment behind her. Hilary presumably wins the democratic nomination and 2012 looks more like 2016.

The book “Game Change” quotes someone as saying, “I’m undecided because I don’t want a Muslim President, but I also fear that if McCain dies in office, Palin will become President.”

It wouldn't have mattered. The Democrat would've slaughtered in 2012 if McCain won in 2008.
