I'd be fine with Medicare for extra if it was able to truly undermine the for profit health insurance scam. Generally speaking, healthier higher income people often get insurance through their employer via for profit companies and were, until not to long ago, pleased with that insurance, and would likely be hesitant to change (of course this somewhat depends on the details - if the rates were based on a percentage of one's income, the top 20-30% opting for private insurance for non-elective procedures would be more problematic). If that for profit system stays intact, there's no higher income people to subsidize the lower income poorer people and thus the system becomes more expensive.

Maybe as a stepping stone this is ok, but ultimately the entire private health insurance racket needs to be rendered completely irrelevant.

My opinion is basically this:

How do you get political support for it?  One of the major reasons healthcare is such a difficult nut to crack politically is that the current system hides the costs of coverage. One of the things the ACA did that in theory would have helped shed some light on it is the requirement to report the total cost of health coverage on the W-2, but because so few people do their own taxes, I doubt many realize that it's on there because its obscured under a Box 12 code that doesn't even affect your taxes.

I would rather vote for HR 676 aka the Medicare For All bill that's in the house right now. Medicare Extra For All is, at best, a half measure.
