Has a candidate for VP ever been selected twice? Example: Joe Smith for VP 2008. Joe Smith for VP 2012. I don't think it has ever happened but does anyone know for sure?

You mean with two separate presidential nominees?

Yes. Example: John Doe for President and Joe Smith for VP 2008. Sam Somebody for President and Joe Smith for VP 2012.

Adlai E. Stevenson was Grover Cleveland's VP from 1893-1897 and was the VP nominee under Bryan in 1900

George Clinton served as VP under Jefferson during his  second term and under Madison during his first term.

John C Calhoun also served under two different Presidents, JQ Adams and Jackson.  (Although the first election was a bit of an anomole, Calhoun was basically the only VP candidate in a race with 4 Pres candidates).

And Thomas A. Hendricks ran with Tilden in 1876 and with Cleveland in 1888!  So both of Cleveland's VP's ran with other presidential candiates.

Finally George Clinton was elected VP with Jefferson in 1804 and Monroe in 1808.

I wouldn't count Calhoun even tho he served under two different presidents as he ran an independent campaign for VP in 1824, getting votes from both Adams and Jackson electors.

It should be noted that this was the grandfather of the Adlai E. Stevenson that ran against Dwight Eisenhower in the 1950's.

But choosing the same VP is rare. Stevenson chose different men each time, just as Nixon chose Henry Cabot Lodge in 1960, and Ted Agnew in 1969 and 1972.

Charles Fairbanks too.
He ran with TR in 1904 and ran again with Charles Hughes in 1916

It has happened many times with third parties.  Gus Hall and Angela Davis ran together about 4 times under the Communist Party nomination.

Charles Fairbanks was selected twice in 1904 and 1916. In 1916 both he and Vice President Marshall came from Indiana, a crucial swing state in 1916.
