Kerry 60-74. Don't know the specifics. Any way to find out?

Where'd you find 60-74? Also, what county, what precinct?

Dilworth Elementary, San Jose, Santa Clara County (Is this the correct identification?)

It's not.  It's your polling place.

To look up your precinct, go to SmartVoter, and select 2004.  The example I looked up (San Jose City Hall) was Precinct 1337; it should be formatted like that.

Did you choose 2004?  The Santa Clara County file skips from 1112 to 1114...

Not sure what to tell you...no such precinct in 2004.  You might have to call the county and ask or something.
