Shannon County, SD:  84.62% Kerry
Sheridan County, NE:  82.19% Bush

Cassia and Blaine, ID have a margin of 88.46% difference.  That's a little bigger than San Miguel/Montrose.

Wasn't San Miguel County Nader's best in 2000 IIRC?

Yep.  San Miguel was 17.20% Nader.  It was followed by Missoula, MT (15.03%); Grand, UT (14.94%); Mendocino, CA (14.68%); and Hampshire, MA (14.59%).
