Atlas data even where they're blatantly wrong (Hancock IN). Excludes counties still shown as blank in Indiana and Arkansas (not Dave's fault - the state sites show the same thing. Due to how elections are reported in these states apparently.)

Shorter list first - counties that voted for Kerry McCain:

2000: Carter, Bath, Magoffin KY
1988: Clay, Lauderdale TN, Lawrence, Mississippi, Hempstead AR
1984: Benton MS, Clay, Randolph, Poinsett, Monroe, Bradley, Little River AR, Assumption LA
1972: Fayette, Washington, Beaver PA, Buchanan, Dickenson VA, Brooke, Fayette, Mingo WV, Webster GA, Pike KY, Van Buren, Grundy, Overton, Smith, Trousdale, Humphreys, Benton, Lake TN, Jackson, Lincoln, Clark AR, Pointe Coupee LA
pre 1960: Logan WV, Breathitt, Knott, Floyd KY, Stewart*, Perry* TN

* voted for George Wallace in 1968.

Trinity CA voted for Perot in 1992. Jefferson AL voted for Wallace in 1968. Oktibbeha MS voted for the unpledged slate in 1960 and for Wallace in 1968. Carson City independent city technically never voted democratic before - the area now covered by the independent city was still called Ormsby County (which included a smaller, non-independent Carson City) back in 1964.

EDIT: Updated to reflect final result. Flips:
to Obama: Warren, Madison NY, Madison, Spencer IN (I think these two were missing from Atlas at the time), Henry, Grundy IL, Pennington MN, Kent MD, Chesapeake, VA, Gallatin MT, Salt Lake UT, Fresno CA.
to McCain (removed from list): Hancock IN (was an error in original reporting), Jerauld SD (IIRC this was actually one of the two election night ties, which I'd counted as Obama gains), Spokane WA (I also removed Stevens and added Whitman, but that was obviously just a printing error on my part in the original list).

This is establishing "voting for the losing candidate" streaks then?

Ah yes. Theoretically. Except as there isn't a single county in the nation that voted for Dole, Kerry and McCain the streaks aren't particularly long...

Logan WV last voted GOP in 1928. Perry TN last voted GOP in 1920. Breathitt, Knott, and Floyd KY and Stewart, TN all voted GOP for the first time ever (at least as far back as the Atlas goes).

Then why do some of them go back further? That's what confusing me and prompted me to ask the question...or does current winner mean party that won them this year? Because that makes some kind of sense I guess.

The latter. If a county voted Democratic for the first time in many years, he's listing the last time the county voted Democratic before this year.

Ugh. (Although I thought this was probably the case in TN.) How far does the Atlas go back for these states? Same question for Illinois, too.

My bad for McLean - edited.

Illinois has county results back to 1892. Tennessee back to 1912.

Breathitt County, KY voted for Taft in 1908. For the others, it was the first time ever.
